As part of your responsibilities, you will be required to conduct initial and follow-up assessments for our clients. These assessments are critical for developing care plans and obtaining medical aid authorizations. Below is a detailed outline of the assessment process, documentation requirements, and important guidelines to follow.
Assessment & Documentation Process
1. Assessment Notification and Information
For every assessment, you will receive an email from our coordination team.
The email will include the care recipient’s (client’s/patient’s):
Full name and surname
Residential address
Contact details of the next of kin or family member
Contact details of assigned caregivers (if applicable)
2. Assessment Form – JotForm Link
A link to a JotForm assessment form will be provided in the email.
This form must be completed thoroughly and accurately.
Information to be captured includes:
Personal background and history
Detailed medical conditions and diagnoses
A complete list of medications currently being taken
? Important: Please take clear photos of all medications (including packaging and labels) during the assessment. This is crucial for proper care planning and medical aid authorization.
Summary Report for Internal Use
In addition to the JotForm form, we require a summary report to be submitted in either PDF or Word format.
This summary is essential for our internal understanding and to support communication with the family. It should be written clearly and professionally, providing deeper insight into the client’s environment and any important social or logistical considerations.
The summary should include:
Observations about the home environment (cleanliness, safety concerns, accessibility)
Financial difficulties indicated by the family, if mentioned
Family dynamics that may impact care delivery
Please be mindful and respectful in your wording (e.g., "Complex family dynamics noted", or "Siblings appear to have differing opinions on care approach")
Red flags or watchouts (e.g., potential neglect, emotional stress of primary caregiver, unsupportive family structures)
Any special considerations we need to be aware of when planning the client’s care
Submission Guidelines
The JotForm must be submitted immediately after the assessment is completed.
The PDF/Word summary should be submitted via email to the admin team within 24 hours of the visit.
