At CareChamp our Registered Nurses (RNs / Sisters) conduct the initial “home assessment and care plan creation” for the care and benefit of our patients. This initial visit also includes the Onboarding of the selected Caregiver Champs.
Furthermore, our Professional Nurses can also be booked for (follow-up) home visits for specific nursing procedures such as:
Wound-care
Catheter care
Vital signs
Administration of medication
Nasogastric tube feeding
Oxygen therapy
Homecare Plan Assessment:
- Going out to the patient's house to identify the plan of care and also provide advice on improving the lives of elders.

Step 1
Once the assessment is completed the registered nurse must communicate to carers separately for at least 10-15 minutes to make sure they are clear of what is required and what assistance is needed - the registered nurse is not familiar with the carers, this 10 minutes will also give her a bit of time to evaluate which carer would be best suitable based on questions she asks them and knowledge around the certain requirements needed.
Step 2
The registered nurse must send us all caregiver duties/tasks and times requested by the client - this completed checklist will instruct the caregiver to what requirements has to be followed.
Step 3
