- Hepatitis Booster certificate
- Police clearance certificate ( CareChamp will book you for your Criminal and Credit Clearance as well as your annual clearance checks ).
- Proof of residence
- Proof of banking details
- Certified copy of ID
- Clear Picture ( in uniform, waist up, no filter used )
- Personality Traits
- 30s Video of yourself ( short overview of yourself on why you became a caregiver and what you strive to be in 5 years or so )
- Once one of our Recruiting Agents has reviewed your application and you meet our criteria and have passed your assessment and reference checks we will send you an Independent Care Provider Agreement to sign and send you a welcome email as well as onboarding.
